Dear Mr Thomas

I am writing to make a Freedom of Information request for information generated from your complaints management system from 1 January 2005 up to the time this request is answered. Specifically, I seek:

1. A listing of all complaints received and their file number
2. The date each complaint was received
3. The date each complaint was acknowledged
4. The public authority the complaint was made against
5. The disposition of the case
6. If a case was closed, the date and reason for closure
7. If a Decision Notice was issued, a copy of the full text Decision Notice.
8. If a Decision Notice has already been issued, all correspondence including emails and letters related the ICO’s decision NOT to publish the Decision Notices.

My preferred format to receive this information is by electronic database. I imagine this is also the easiest method for you, but if these reports are only available in manual form then copies are acceptable. In order to expedite this request and avoid Data Protection issues, I am only interested in the names of the plaintiffs in cases where they are acting as part of an organisation or company. However, the names of all public authorities should be released as a matter of course as these are public bodies rather than private individuals.

I understand that under the act, I am entitled to a response within 20 working days. I would be grateful if you could confirm in writing that you have received this request. If you have any questions or need further clarification please feel free to contact me via telephone, email or post.
